Interrater Reliability of FLACC Behavioral Pain Assessment Scale among Nurses in Recovery Room

Abstract
Purpose: This study was to measure the interrater reliability of FLACC Behavioral Pain Assessment Scale (FLACC Scale) made by nurses working in a recovery room.

Methods: The participants were 18 nurses and 101 patients in an P university hospital in Yangsan city during July 1 to 31, 2016. We divided six groups with three nurses in a group. Each group had one nurse who had less than two year working
career, one nurse who had two to five year working career, and one nurse who had six year and more working career. The interrater reliability data were analyzed by intraclass correlation coefficient (ICC) and 95% confidence interval (CI).

Results: Overall interrater reliability of FLACC Scale with ICC was .83 (95% CI: .77-.87), ICC for sub-items ranged from .61 (95% CI: .52-.70) for activity to .75 (95% CI: .69-.81) for face. The overall and sub-items¡¯ ICCs were the highest between nurse less than two year working career and nurse six year and more working career.

Conclusion: Overall interrater reliability for measuring FLACC Scale was high, but less satisfactory in activity and consolability items, and nurses who had two to five year working career. It is needed more training on how to measure FLACC Scale to improve the interrater reliability for the items showed relatively low interrater reliability such as activity and consolability, and for nurses with two to five year working career.
